PERSON-CENTERED SUPPORT

In compliance with COMAR 10.22.05, the Person-Centered Plan will be utilized to assist the participants supported by NPLH Service Group in becoming more independent. As part of the person-centered planning process, NPLH Service Group will meet with the participant to discuss the following areas:

2. Things You Need to Know About Me
3. My Goals
4. People Who Are Important to Me
5. How I Want to Socialize
6. Where I Want to Worship
7. My Staffing Choices and Preferences
8. What Works Best For Me

Addressing these key areas gives NPLH Service Group a better understanding of the person supported, affording NPLH Service Group the opportunity to provide high quality supports.
